Summary: A Multicenter, Randomized, Parallel-Group, Double-Blind, Placebo-Controlled Flexible-Dose Study to Assess the Efficacy and Safety of Viagra® (Sildenafil Citrate) in Men with Erectile Dysfunction (ED) who do not Self Identify.
Purpose: To see if Viagra® (Sildenafil Citrate) is helpful to men with early changes in erectile/sexual function.
Description: This is a 12 week study. During the first phase of the study (first 8 weeks), one group will receive placebo and the other group Viagra® (Sildenafil Citrate). During the second phase (remaining 4 weeks) all subjects will receive Viagra® (Sildenafil Citrate). The subjects will fill out questionnaries regarding sexual function and will have their blood pressure and heart rate measured at each visit, plus a physical exam at the screening visit. Any adverse event will be recorded during the study.
Eligibility Inclusion Criteria:
  • Male aged 30 years or older
  • Answer “no” or “unsure” to the question “Do you have erectile dysfunction?” at screening
  • rectile Function domain score is 25 or lower at screening
  • “ Be in an active stable sexual relationship fot he duration of the study
  • “ Have a history of at least one of the following risk factors: hypertension, hypercholesterolemia, smoking, metabolic syndrome, coronary artery disease, stable heart disease diagnosed more than 3 months ago, diabetes, depression, anxiety disorders, lower urinary tract symptoms or obesity.
Exclusion Criteria:
  • Resting sitting and/or standing hypotension (low blood pressure) or uncontrolled hypertension (high blood pressure).
  • Orthostatic hypotension (significant blood pressure drop when standing).
  • Significant heart disease in the last 3 months such as: heart failure, myocardial infarction (heart attack), unstable angina (chest pain), stroke, transient ischemic attack (TIA), symptomatic or clinically significant cardiac arrhythmias (heart rhythm problems).
  • Currently taking and/or likely to be treated with nitrates or nitric oxide donors (Nitroglycerin Minitran, Isosorbide dinitrate, Isosorbide mononitrate, Sodium nitroprusside) in any form on either a regular or intermittent basis.
  • Any relevant clinically significant abnormalities on the screening physical examination.
  • Have previously taken more than 6 pills of a phosphodiesterase type 5 (PDE-5) inhibitor such as VIAGRA (Sildenafil), CIALIS (Tadalafil) or LEVITRA (Vardenafil).
  • Have a known hypersensitivity to sildenafil citrate or any part of the study medication; or have had any intolerable side effects to sildenafil citrate.
  • Have undergone any type of radical prostatectomy (surgical removal of the prostate), radiation for the treatment of prostate cancer or any radical pelvic surgery.
  • Currently using any commercially available treatments or devices (including penile implants, intracavernosal or intrapenile injections or vacuum erection devices) for ED. Such treatments may not be used during the study. Subjects currently receiving testosterone are eligible provided the dosage is stable throughout the study.
  • Have a known history of any severe liver and/or kidney problems, and/or a known history of retinitis pigmentosa (eye disease that affects the retina, which is the inner layer of the eye).
  • Have a spinal cord injury or neurodegenerative disorder (e.g., multiple sclerosis).
  • Have a history of NAION (Non-arteritic Anterior Ischemic Optic Neuropathy, a cause of blindness).
  • Receiving concomitant treatment with ritonavir, saquinavir, ketoconazole, itraconazole, erythromycin or cimetidine.
  • Current or anticipated treatment with any investigational agent during the trial period or within 6 weeks prior to enrollment.
  • Have any medical or psychological condition or social circumstances that would affect your ability to participate  in the study, in the opinion of the investigator.
  • Have donated blood within 4 weeks prior to screening, or you intend on donating blood or blood products during the period of the study or within one month following completion of the study.
  • In the opinion of the investigator, abuse alcohol or drugs.
  • In the opinion of the investigator, are unable or unwilling to consistently and accurately complete the Event Log/Sexual Encounter Profile (SEP)/Erection Hardness Grading Scale (EHGS) and answer the required questionnaires.
  • In the opinion of the investigator, are unlikely to complete the study for whatever reason including planned elective surgery.
Monetary Compensation: Patients will get U$10 for completing visit 1, and then U$30 for each of the following visits, for a total of U$160 if they complete all study visits

Nitric oxide then activates the enzyme guanylate cyclase, which results in increased levels of cyclic guanosine monophosphate (cGMP)

This produces smooth muscle relaxation in the corpus cavernosum and allowing inflow of blood into the erectile tissue.

Sildenafil Citrate enhances the effect of nitric oxide by inhibiting PDE5, which is responsible for degradation of cGMP in the corpus cavernosum.

When sexual stimulation causes local release of nitric oxide, inhibition of PDE5 by Sildenafil Citrate causes increased levels of cGMP in the corpus cavernosum of the penis, resulting in smooth muscle relaxation and inflow of blood to the corpus cavernosum.
